7 AAC 57.030(c)(9)(C) - Application for license; biennial renewal.

The facility’s State of Alaska Business license was not renewed prior to the expiration date – December 31, 2025. Regulation: An applicant for a provisional or biennial license under AS 47.32 and this chapter shall submit a completed application on a form supplied by the department and shall provide the following information as applicable to the type of child care facility: a copy of a business license as required under AS 43.70; and Cited at a unannounced Follow Up inspection. 7 AAC 57.230(d) - Records.

Application materials were not on file for three employees. Regulation: A child care facility shall maintain personnel records for employees and caregivers. For employees, these records must include starting and ending dates of employment, application materials, annual and interim performance evaluations, orientation and training documentation, and personnel action memoranda of commendation or reprimand. 7 AAC 57.350(b) - Orientation and Training

Caregiver did not complete CPR training within three months from the individual's date of hire. Regulation: Unless otherwise specified in this section, the orientation and training required in (a) of this section must be completed within three months from the individual's date of hire. 7 AAC 57.350(e)(1) - Orientation and Training

A caregiver did not complete pediatric CPR and first aid training within three months from the individual's date of hire when a caregiver employed since May 27, 2025, had not taken a course for pediatric CPR and FA certification. Regulation: A child care facility shall ensure that each of its caregivers has a valid certification for pediatric first aid and pediatric CPR. If certification courses are not available in the community in which the facility is located, a caregiver may receive instruction in pediatric first aid and CPR procedures through distance learning, training videos, and other materials approved by the department; and Cited at a announced Renewal inspection. 7 AAC 57.500(f) - Supervision of children.

Not all caregivers present in the classroom were signed in on caregiver attendance records during onsite investigation conducted 5/8/2025 and 5/23/2025. Regulation: A child care facility shall maintain caregiver and child attendance records that reflect the time caregivers are present and children are in care. 7 AAC 10.1045(4) - Universal precautions.

A caregiver was wearing the same gloves when they handled a mouthed toy from a child, filled another child's water bottle, and reached into a large bag of jelly beans to disperse jellybeans to children's wall pockets to take home at the end of the day. Regulation: An entity shall take precautions to reduce risk against the spread of a communicable, contagious, or infectious disease that could pose a significant threat to the health, safety, or welfare of adults or children in care.
