7 AAC 10.1050(c)(2) - Caregiver Hygiene

Caregiver completed diapering and did not wash hands before interacting with infants. 7 AAC 10.1060(e)(6) - Additional provisions for entities licensed to provide care for children.

Caregiver failed to clean and sanitize diapering surface after changing a diaper. Regulation: In addition to the requirements of (d) of this section, an entity shall ensure that each diaper changing surface is cleaned and sanitized after each use; if a single-use, disposable cover is placed on the diapering surface before diapering, the entity shall ensure that the cover is disposed of immediately after diapering; the use of a single-use, disposable cover does not preclude or substitute for cleaning and sanitizing the surface or area after each use even if the surface or area is not visibly contaminated; Cited at a unannounced Investigation inspection. 7 AAC 10.1060(e)(7) - Additional provisions for entities licensed to provide care for children.

The container for soiled diapers had no tight-fitting lid and was overflowing and placed on the diaper changing counter next to hand washing sink. Regulation: In addition to the requirements of (d) of this section, an entity shall ensure that after a soiled disposable diaper is removed, it is folded inward and resealed before disposal into a container described in (5)(B) of this subsection; Cited at a unannounced Investigation inspection. Single use towels were not accessible to children in the Toddler Room. Regulation: Except for an assisted living home providing service for two or fewer residents, the entity shall ensure that each handsink is used only for its designated purpose and is equipped with soap and at least one accessible single-use towel dispenser or heated air hand-drying device; the entity shall ensure that towels are discarded after each use; Cited at a unannounced Investigation inspection. 7 AAC 57.505(c)(4) - Child-to-caregiver ratios.

On July 17, 2025, child-to-caregiver ratios were not met in the Toddler Room or the Primary room where they both had mixed age groups. In the Toddler Room, there were six children present with one caregiver and the ratio should have been 1 caregiver to 5 children. In the Primary Room there were seven children in the classroom upon arrival with one caregiver and the ratio should have been 1 caregiver to 6 children. In addition, attendance records were reviewed from June 9, 2025 - July 17, 2025, which totaled 16 days that children were in care and mixed ages were present in the Toddler Room and the Primary Room. Child-to-caregiver ratios for the youngest child was not applied in the Toddler Room for 9 out of 16 days, and the child-to-caregiver ratios for the youngest child was not applied in the Primary Room 13 out of 16 days. Regulation: For a child care center, the child-to-caregiver ratios for the youngest child apply when infants, toddlers, and preschoolers are in a mixed age group. AS 47.32.140(b) - Centralized Licensing and Related Administrative Procedures.Purpose and applicability.

The entity did not cure or ensure violations will not recur as stated in the Plans of Correction for inspections conducted on the following dates: October 26, 2021, March 23, 2022, September 21, 2022 and May 2, 2023. Plans of Corrections submitted by Administrator Kathryn Torres, all stated ratio violations would be cured however, during the current on-site investigation there were not enough caregivers for the number of children present in both the Toddler and Primary classrooms. In addition, attendance records were reviewed from June 9, 2025 - July 17, 2025, which totaled 16 days that children were in care and mixed ages were present in the Toddler Room and the Primary Room. Child-to-caregiver ratios for the youngest child was not applied in the Toddler Room for 9 out of 16 days, and the child-to-caregiver ratios for the youngest child was not applied in the Primary Room 13 out of 16 days. Regulation: An entity receiving a notice under (a) of this section, or a notice under AS 47.32.130(b)(2) that contains the information specified in AS 47.32.130(b)(2)(B), shall submit a plan of correction to the applicable department for approval. Once it has cured its violations, the entity shall submit to that department an allegation of compliance. Upon receipt of the allegation of compliance, the applicable department may conduct a follow-up investigation or inspection to determine compliance. The department that provides notice of a violation may take one or more enforcement actions under (d) and (f) of this section regardless of whether the entity achieves compliance under this subsection.
