7 AAC 10.1095(a)(3) - Toxic substances; poisonous plants.

7 AAC 57.350(b) - Orientation and Training

The orientation and training required in (a) of this section was not completed within three months from the individual's date of hire for all caregivers. Regulation: Unless otherwise specified in this section, the orientation and training required in (a) of this section must be completed within three months from the individual's date of hire. 7 AAC 57.310(d)(1) - Qualifications and responsibilities of employees and other individuals in a child care facility.

Not all caregivers had at least three written references on file before allowing the caregiver to have contact with children in care. Regulation: A child care facility with one or more employees or other caregivers shall obtain a completed application for employment or other work from each prospective employee or caregiver before allowing the applicant to have contact with children in care. The completed application must provide the information necessary to determine whether the applicant has the qualifications required under this section. In addition, the facility shall obtain at least three written references on a prospective employee, at least two of which are from individuals unrelated to the applicant, and at least one reference on any other caregiver, from someone unrelated to the caregiver. 7 AAC 57.350(f) - Orientation and Training

Not all caregivers have received at least 24 hours of annual training relevant to child care and development, based on the individual's hire date with the facility. Regulation: Except as provided in the training requirements of 7 AAC 57.300 for the administrator, a child care center, child care group home, or child care home shall ensure that each administrator, and caregiver receives at least 24 hours of annual training relevant to child care and development. At least one hour of annual training must address a health and safety training topic in (a)(4) of this section based on the individual's hire date with the facility. Except as provided in (j) of this section, the 24 hours of annual training must be in addition to the orientation and training required under (a) of this section and the training in first aid and CPR required under (e) of this section. An administrator or caregiver may satisfy the annual training requirements of this subsection by obtaining college credit relevant to child care and development if the department approves the credit. 7 AAC 10.900(b)(3)(A) - Background check

Not all caregivers had a valid background check prior to having contact with children. Regulation: The provisions of 7 AAC 10.900 - 7 AAC 10.990 apply to a provider seeking approval from an oversight division to provide services or receive payments from the department. Each individual who is to be associated with a provider in a manner described in this subsection must have a valid background check conducted under 7 AAC 10.900 - 7 AAC 10.990 if that individual is 16 years of age or older and will be associated with the provider as an employee, an independent contractor, an apprentice, an unsupervised volunteer, or a board member if that individual has an employee, an independent contractor, an apprentice, an unsupervised volunteer, or a board member if that individual has regular contact with recipients of services; Cited at a announced Renewal inspection. 7 AAC 57.230(d) - Records.

An annual or interim performance evaluation was not on file for all caregivers. Regulation: A child care facility shall maintain personnel records for employees and caregivers. For employees, these records must include starting and ending dates of employment, application materials, annual and interim performance evaluations, orientation and training documentation, and personnel action memoranda of commendation or reprimand.
