Children in the school age room were observed having screen time during their rest period. This was corrected when children went to the table to begin another activity.

746.2909(3) - Arrangement of Napping Equipment -Provides Sufficient Walking and Work Space

Mats in the after-school classroom were to be observed to close not allowing from proper walking space during rest time.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

Based on the preponderance of information obtained during the investigation, there is sufficient evidence to support a caregiver did not use good judgement when redirecting a child's behavior.

746.2403(6) - Infant Care Area - Not Used as a Passageway

Four children ages 21 months-23 months old were observed using the infant room as a passage way to the classroom used to care for them during the inspection.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

Two 2-year-olds were observed unsupervised in the infant classroom as the caregiver was at the front door.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

One caregiver was observed caring for 17 children with a specified age group of three years during the inspection.

Based on the DFPS investigation, there is sufficient evidence to suggest that supervision was not maintained when a child was able to wander away from the group during a field trip.

There was sufficient evidence that a classroom was out of ratio when an injury occurred.
