Two out of ten child files evaluated were missing an address for their listed emergency contact on their enrollment form.

One child was missing a copy of immunization records upon enrollment. Two children were in need of updated immunization records.

Upon review of video footage, infants were observed opening cabinet doors under the sink area 5 separate times without a caregiver intervening.

746.2401(2) - Basic Requirements for Infants - Individual Attention

Upon reviewing video footage, infant caregivers were observed sitting in the rocking chairs in the classroom during the day and not engaging with the infants when awake.

746.1203(3)(D) - Personal use of electronic devices, such as cell phones, MP3 players, tablets, and video games

Upon review of video footage, caregivers were observed sitting and looking at their personal cell phones throughout the day.

746.2417(2)(B) - Activities for Infants - Not in Restrictive Device for more than 15 Minutes

Based on video footage reviewed, two infants were in swings for 17 minutes and 40 minutes.

746.613(b) - Immunization Records, Exceptions, and Exemptions Current

Three out of ten child immunization records reviewed did not have current records on file. One child is currently 2 yrs of age and the last updated record was at 6 months. One child is currently 8 months old and is missing their 3 and 5 months vaccinations. One child is 6 months old and is missing their 5 month vaccinations.
