746.1311(e) - Director Annual Training - Additional required topics

While reviewing Director's records the following required trainings hazardous materials, allergic reactions, and medication administration were missing for the annual year 3/20/2024-3/20/2025.

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

The most current inspection was conducted on 3/15/2024 which expired on 3/15/2025.

746.1309(e) - Documented Annual Training- Required Content

One out of the two employee records reviewed was missing the following required trainings building safety, emergency preparedness, and communicable diseases for the annual year 7/1/2024 to 7/1/2025.

746.1315(a) - Each Caregiver and the Director Must Have Current Training in pediatric first aid with rescue breathing and choking.

One out of the three employee records reviewed was missing the most current CPR/First Aid Certificate which expired 12/2025.

746.303(b) - Report Number of Employees

A review of the Employees Who Left Employment in 2025 data from January 20, 2026, determined the center had not reported the number of employees who left employment in 2025 by January 15, 2026, as required.

746.5205(1) - Fire Drill Monthly - Exit in 3 Minutes

The last fire drill practice was done on 1/13/2025. Fire drill practice for Feburary was done during inspection.

746.1309(e) - Documented Annual Training- Required Content

In the health and safety audit, it was found that two out the three of the employee records examined lacked the following annual training: hazardous materials, building safety, medication administration, allergic reactions, and communicable diseases.

746.3401(c)(1) - Annual Sanitation Inspection - Inspection not available, you must obtain documentation stating inspection not available

The last sanitation inspection expired on February 29, 2024, and there was no evidence available to show that an appointment had been arranged with the health department for an inspection to occur before March 31, 2025.
