Evidence obtained during the investigation showed that a caregiver did not demonstrate competency when they picked a child up in a way that did not follow the daycare's procedures. This occurred despite the employee having received training on this topic prior to the incident.

746.3505(e) - Diapering Procedures - Sanitizing Diaper Changing Area

A fabric cover was observed on a changing pad that cannot be sanitized between each child.

746.5205(1) - Fire Drill Monthly - Exit in 3 Minutes

Evidence obtained during the investigation showed that the operation has not been conducting the required drills.

746.3407(12) - Mark Cleaning Supplies, Toxic Materials; Keep Separate from Food and Inaccessible to Children

Cleaning supplies were stored in a closet that the door was open to and was accessible to children in care.

746.1301(a)(5)(A) - Training Requirements for Employees, Caregivers, and Directors - 24 Annual training

Five of the seven staff files reviewed did not have at least the required 24 hours of annual training.

746.2419(3) - Requirements for Feeding Infants - Never Prop Bottles

Evidence obtained during the investigation shows that bottles have been propped for infants during feeding times instead of being held by an adult. The infants were not old enough to hold the bottle themselves.

746.2419(4) - Requirements for Feeding Infants - Regular Meals and Snacks

Based on evidence obtained dung the investigation, two children were not fed according to their normal mealtimes.

746.2805(3) - Prohibited Punishments - Grabbing and Pulling

Based on the evidence obtained during the investigation, a caregiver picked up a child up by one arm causing the child's elbow to become dislocated.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

Two caregivers present at the operation did not have eligible background checks and were in classrooms with children in care.

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

The most recent fire inspection is from February 2023.
