746.1203(10) - Responsibilities of Caregivers - Share Information with Incoming Caregivers

Based on the evidence obtained during the investigation, a caregiver did not share information with incoming caregivers taking responsibility of her group of children. One child sustained an injury, minutes before transitioning to a new classroom with new caregivers. None of the information about sustaining an injury was communicated to the incoming caregivers so the information could not be communicated to the parents received timely information about the injury.

746.1205(a)(3) - Responsibilities of Caregivers - Know children's names and ages

Observations were made at inspection with a classroom being supervised by 3 caregivers. None of the three caregivers were knowledgable of the number of children they were responsible for. Additionally, they could not provide an accurate information to child care regulation on the ages of the children in the group. The list in the classroom needs to be updated to include all of the children's names and ages.

746.2805(3) - Prohibited Punishments - Grabbing and Pulling

Based on the evidence obtained during the investigation, a caregiver grabbed a child in a rough manner by the arm on multiple occasions related to discipline.

745.635(3) - AP Background ck determination - If provisionally eligible w/conditions or eligible w/conditions you must restrict duties per conditions/restrictions

A caregiver with conditions on their background check was observed at inspection being the only caregiver in the classroom when conditions specify that they can't be left alone with children.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

It was found during the investigation that children were outside the supervision of caregivers for at least 5 minutes.

746.2205(a)(4) - Written Activity Plan - No Long Waiting Periods

It was found that the activity plans contain prolonged periods of up to 2 hours during which children are expected to sit at the table.
