746.3407 - Maintenance of Building, Grounds and Equipment

An air conditioner unit had dust accumulation on the vent and dead ants around the unit, dead roaches were on the floor of the children's bathroom, and a door was unlocked to a classroom being used as a storage area.

746.3809 - Medication No Longer Used

Medication was stored for a child enrolled at the operation and was expired.

746.2805(3) - Prohibited Punishments - Grabbing and Pulling

During the investigation process, it was determined that a caregiver pulled child's arm and hair. *Dir addressed the situation as soon as it was brought to their attention and terminated the caregiver. *

746.1203(5) - Responsibilities of Caregivers - Interact positively with children

A caregiver made sarcastic comments when a child returned to the operation. *Director addressed the situation when it was brought to their attention and terminated the caregiver. *

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

Based upon information gathered as a result of this investigation, documentation, video footage and interviews conducted as it was determined a child was found in classroom #4 unsupervised as the child went through the double doors from classroom #3.

Two of the ten children were missing shots at the time of the inspection.

746.605(10) - Required Admission Information - Physician Information

One of the ten children files was missing contact information for their emergency care or physician. NOTE: Corrected at inspection.
