746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

The caregiver wrapped a plastic bag around the child?s hand, placed an ice pack inside the bag, and used a hair band to secure it in place around the child's wrist.

746.307(b)(1) - AP Parental Communication - Injury Requires Treatment by Health Care Professional or Hospitalization

A child's relative arrived to pick up the child and was informed of the child's fingernail being removed after the finger was shut in the door. When the relative asked if the parents had been contacted, the caregiver said they had not yet spoken with the parents. The relative informed the parents.

746.3417(3) - Children Handwashing After Toileting or Diapering

An infant was moved directly to the group feeding table after having their diaper changed without having their hands washed. This was corrected at inspection when the caregiver was advised to remove the child and wash their hands.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

The infant room had six children, five of which were 11 months or under, with one caregiver. This was corrected at inspection when a second caregiver was brought into the room.

746.2207(a) - Screen Time Activities - Not Used for Under Two

Seven children whose ages ranged from 18 to 23 months were observed having screen time. This was corrected at inspection when the caregiver opened up the centers in the room.

746.2426(a)(1) - Infant May Not Sleep in a Restrictive Device

An infant was observed sleeping in a bouncer and another was observed sleeping in a swing. This was corrected at inspection when the children were moved to their cribs.

746.1613(a)(5) - Naptime Child/Caregiver Ratio - Additional Employees Required

There were two rooms running with nap time ratios and one caregiver in the infant room with no one else was in the building. This was corrected at inspection when a controlling person was called in.

745.635(3) - AP Background ck determination - If provisionally eligible w/conditions or eligible w/conditions you must restrict duties per conditions/restrictions

A provisional employee was observed in charge of a group of children by herself.

746.1003(1) - Director Responsibilities - Operate in Compliance

Based on the information gathered during the course of the investigation it was determined that minimum standards regarding ratios and supervision were regularly disregarded due to staffing issues. No effort was made to adjust the student population in order to meet minimum standards.

746.2803(4) - Discipline - Positive Methods of Discipline and Guidance

Based on the information gathered during the course of the investigation it was determined that disciplinary techniques used with children have not been positive.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

During the inspection eight children, whose ages ranged from 3 to 17 months, were observed with one caregiver. The specified age group was 0 to 11 months.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

During the inspection two classrooms that were combined for nap were observed in their room without a caregiver present. Another class was left unattended when the caregiver left it to open the front door. This was corrected at inspection when the cook moved to the room and the other teacher returned to the room.

746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

The PreK teacher in classroom 129 stated there were 10 children in care when we verified there were only eight children.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

Based on information gathered, ratios have not been met with the mixed age group.
