746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

Based on video footage reviewed, a caregiver failed to demonstrate good judgment and was observed engaging in multiple inappropriate physical interactions with children. The caregiver was seen grabbing a child in an inappropriate manner and placing the child into a chair, pushing children toward the restroom, and aggressively placing a child onto a nap mat.

Classrooms two and four did not have emergency evacuation and relocation diagrams posted near the entrance or exit of the classroom. The operation came into compliance when the caregivers in rooms two and four posted the emergency evacuation and relocation diagrams near the exits of their classrooms.

There is a tree stump surrounded by black mushrooms on the head start playground two that poses a signifact tripping or fall hazard.

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

The operation did not have a current fire inspection on file the last documented inspection was 4/22/2024.

746.3401(a) - Annual Sanitation Inspection

The operation did not have a current health inspection. The last documented inspection was 5/31/2024.

746.3701 - Safety - Areas Free From Hazards

At the time of the inspection there were multiple hazards observed. At the front gate of the facility, that is an entrance to the facility and a gated play area, the latch is broken. This allows the gate to open freely and leaves a risk for children to be able to leave the area. In the older preschool play area a shed has been replaced. Along the side of the new shed there is a gap between the mulch and shed. The shed is stabilized on cinder blocks and there is a gap underneath the shed. This creates a space a child could crawl into, or an animal could easily burrow into. At an exit gate with steps down there is cracked and jagged concrete and a gap created due to erosion of dirt. This creates a gap between the ground and the bottom of the fence and a tripping hazard for children playing. Multiple water hoses were piled next to the building loosely.

746.2415(b) - AP Cribs - Only Snug Fitting Sheet and Crib Mattress Cover Allowed if Infant Younger than 12 Months of Age

At the time of the inspection a child under 12 months of age was sleeping inside a crib, on their stomach, with a blanket over them. This blanket covered from the child's shoulders to their ankles. The caregiver was seated in the chair directly next to the child's crib.
