746.2409(a)(2) - Safety Requirements for Cribs - Snug Fitting Sheets

Two infants in seperate cribs were observed in cribs with loose fitting sheets. This was corrected when brought to caregiver attention and she fixed th sheets with the sheet clamps.

746.1315(b) - Each Caregiver and the Director Must Have Current Training in pediatric CPR

Staff files reviewed were missing the hands-on skills based training.

746.3407(9) - Floors, ceilings and Walls in Good Repair and Clean

Walls in classrooms had chipped paint and wearing dentures. Floor was coming up in the 18m classroom. Toys and equpment observed not in good repair and missing pieces.

746.1316(a) - Transportation Safety Training -Director and Employee Complete 2 Hours Annually

There was not any documentation of current transportation training.

746.5617(a)(6) - Required Equipment in Vehicles - First-aid Kit

Upon review the first aid kit was not complete and missing tweezers, scissors, and thermometer.

746.3701(1) - Safety - Electrical Outlets Covered

Electrical outlets were not covered properly in two classrooms with children under 5 years old.

746.2407 - Infant Care Area Furnishings and Equipment -Use of Safety Straps

In the toddler room eight children seated in the table chair were not strapped in and their were not straps as required by the manufacturer. In the infant room a child was in a high chair and not strapped in, the highchair had no straps. In the lunch area a child was observed in a highchair with no straps.

746.1309(a) - Documented Annual Training Area - 24 Hours Required

Out of the eight staff files three staff did not have the required trainings.

746.2426(a)(1) - Infant May Not Sleep in a Restrictive Device

A 3 month infant was asleep in his carseat during the inspection.

746.2207(c)(4) - Screen Time Activities - Not Used During Eating or Rest Times

The tv was on during breakfast with two infants under 18 months present.

746.2419(3) - Requirements for Feeding Infants - Never Prop Bottles

During investigation inspection a 3 month infant was strapped in car seat with a bottle propped in his mouth.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

Based on evidence from a DFPS investigation, a caregiver was left outside alone with 25 children that were 2, 3, and 4yrs old.

746.4607(b)(2) - Maximum Height - Under Age 5

Based on evidence from a DFPS investigation, children younger than 5 years of age were playing on active equipment that had a play surface that was higher than 5 feet.

746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

Based on evidence from a DFPS investigation, two caregivers did not know how many children they were responsible for in their groups.

746.1605(1) - Child/Caregiver Ratio - 18 Months Difference in Age

A 26 month old child was counted in ratio in the infant room with the youngest infant present at 2 months old. There were 2 caregivers present with 7 children total.

The operation did not have a current fire inspection
