746.2415(a)(1) - Prohibited Equipment for Infants - Baby Walkers

An infant was observed utilizing a walker with wheels that would allow movement. Walkers are prohibited equipment and would allow infants to move around and have access to materials or may cause a hand injury to other infants during floor time. The deficiency was corrected during the inspection when the staff removed the infant out of the equipment and removed the prohibited equipment out of the classroom with CCR brought it to their attention.

746.4907(a)(1) - Installation and Maintenance of Loose-fill Surfacing - Six Inches if Equipment 5 Ft or Less

The loose-fill surfacing for the swings and Preschool playground had a depth of 1-4 inches under and around the equipment.

746.801(13) - Required Records Maintained and Made Available - Gas Inspection Report

The operation did not have proof of a current gas leak inspection.

746.801(2) - Required Records Maintained and Made Available - Infant Feeding Instructions

Three infants were missing current feeding schedules.

The loose-fill surfacing on the School Age playground had a depth of 1-4 inches under and around the equipment.

746.1315(a) - Each Caregiver and the Director Must Have Current Training in pediatric first aid with rescue breathing and choking.

During a DFPS investigation, one caregiver was found to be missing pediatric first aid training.

During a DFPS investigation, one caregiver was found to be missing pediatric CPR training.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

During an inspection, a caregiver was present with children prior to the operation receiving an eligibility letter from the Centralized Background Check Unit.

745.635(3) - AP Background ck determination - If provisionally eligible w/conditions or eligible w/conditions you must restrict duties per conditions/restrictions

During an inspection, it was observed that a caregiver was not adhering to the conditions on their background check.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

During the inspection, there were 12 children aged two years old in a class with one caregiver.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

This standard was evaluated as part of the investigation and was found to be deficient. Based on review of video, and interviews of children and staff, it was determined a caregiver left a child unsupervised in a classroom. The child left the building where their classroom was located. The child was found by another caregiver on the concrete path between the two buildings of the operation. The child was unsupervised for 6 minutes and 59 seconds.

746.5207(a)(1) - Emergency Evacuation and Relocation Diagram - Floor Plan

Three of the eight rooms did not have emergency evacuation and relocation diagrams posted in the room. This was corrected at inspection when the operation printed out new diagrams and posted them in the rooms.

746.2421(b) - Written Feeding Instructions - Review and Update Every 30 Days

Two of the four infants in care did not have current infant feeding schedules.

746.5205(3) - Sheltering Drills for Lock-Down-4 times in a calendar year

The operation has not conducted a lockdown drill in the past year.
