Two staff were unaware of the age group of the children that they were responsible for.

I observed a caregiver that was being counted in ratio, walk out of the classroom, leaving the children in her room unsupervised. This was corrected when the caregiver walked back into the classroom.

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

Most current fire inspection was not available for review.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

A staff was present, being counted in ratio and supervising a group of children by herself, without the staff being cleared by the background check unit as eligible.
