746.2427(a) - AP Must Place Infants in Face up Sleeping Position or Must Have Completed and Signed Sleep Exception Form for the Infant

Two children were observed sleeping face down. This was corrected when the caregiver rolled the children over on their backs.

746.307(b)(4) - Parental Communication - Situation that Placed a Child at Risk

This is related to a DFPS investigation. The operation did not notify the parent that a child had been left outside of the operation.

746.305(a)(6) - Report Situation Placing Children at Risk

This is related to a DFPS investigation. The operation failed to self-report an incident that placed a child at risk while in care.

746.4309 - Fenced Outdoor Activity Space -Gates Locked

This is related to a DFPS investigation. A gate leading to the outside of the playground did not have a lock. The unlocked gate was accessible to children, allowing children the ability to exit the playground.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

During a DFPS investigation, it was found that a child in care was left on the playground alone for less than 5 minutes.
