746.1309(e) - Documented Annual Training- Required Content

Out of all staff files reviewed, all staff were missing the following trainngs: emergenccy preparedeness, preventing and controlling the spread of communicable diseases, administering medication, preventing and responding to emergencies, understanding building and premises safety, handling, storing and disposing of medication.

746.1309(d) - Documented Annual Training - Caregivers of Children Under 24 Months Required

There are 2 caregivers who rotate being caring for infants who do not have current training in SIDS.

746.3701 - Safety - Areas Free From Hazards

There is an old indoor AC blower in the corner of the playground accessible to the children. There was an old flower pot that was rusted at the bottom and had fallen over (this was removed from the playground.)

746.1601 - Child/ Caregiver Ratio - 13 or More Children

There were a total of 10 toddlers with 1 caregivver. The specified age was 22 months which caused the classroom to be out of ratio by 1. (NOTE: Corrected during inspection as the oldest child was moved into the next classroom.)

746.4601(7) - Safety Requirements for Active Play Equipment - No Pinch, Crush, or Shear Points

There is a Lil Tykes type of structure that has a cracked in the middle section which poses a pinching hazards.

746.1309(c) - Documented Annual Training - 1 hour in Prevention, Recognition and Reporting of child maltreatment

Staff files reviewed shows that they are missing training in child maltreatment.

746.603(a)(4) - Children's Records - Immunizations

Out of children's files reviewed, 1 was missing the needed immunizations.

746.303(b) - Report Number of Employees

A review of the Employees Who Left Employment in 2024 data from January 16, 2025, revealed the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.
