In looking at staff records one employee was lacking the 2912 affidavit in their file.

Based on the information gathered, the operation is not allowing parents to enter and have access into the daycare classrooms.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

A caregiver did not use good judgment when a child's elastic laces were repurposed and tied across the top of both shoes causing bruising to both feet.

746.3417(1) - Children Handwashing Before Eating

3 children observed to enter the classroom at drop off time did not wash their hands prior to eating.

746.2401(4) - Basic Requirements for Infants - Prompt Attention to Needs

An infant child was observed to be in a baby jumper type equipment and crying with their arms inside of the structure and was unable to support themselves. Two caregivers present in the classroom did not tend to the child's physical needs promptly.

746.2407 - Infant Care Area Furnishings and Equipment -Use of Safety Straps

Four children were observed sitting in high chairs with the safety straps missing not buckled inside their seats.

746.5625(a) - Electronic Child Safety Alarm - is equipped in each vehicle

Two vehicles used to transport children do not have an electronic child safety alarm that allows for the employees to turn off at the rear of the vehicles.
