Three of six children's files were audited and did not have current immunization records on file.

Based on the information gathered, it was determined that the caregiver did not use good judgment when she used her cell phone to record and livestream on TikTok while caring for children.

746.1203(3)(D) - Personal use of electronic devices, such as cell phones, MP3 players, tablets, and video games

Based on the information gathered, it was determined that the caregiver was on her phone while responsible for a group of children.

746.4503(b) - Cots, Beds, Mats Labeled With Child's Name

Cots in the toddler and Pre-K room are not labeled with the child's name.

746.2911 - Adequate Lighting During Naptime

There was not adequate lighting in the Pre-K room. Note: This was corrected at inspection when an additional light was turned on in the next room, allowing light to come into the room.

There is no emergency contact information in the bus for the parents of the children being transported.

The Children's Product Certification is not posted for staff and parents to review any products that have been recalled. This was corrected at inspection and posted.

Caregiver did not wash a child's hands after diaper changing. This was corrected at inspection when the caregiver was prompted to wash the child's hands.
