746.1309(a) - Documented Annual Training Area - 24 Hours Required

Five of ten staff files reviewed were lacking training for Handling, storing and disposing hazardous materials and/or Preventing, controlling, responding to emergencies due to food or allergic reactions.

746.3701(5) - Safety - Play Materials and Equipment Safe

It was observed that the school age playground equipments plastic was broken and split exposing rusted metal that could cause an injury to a child. The opeartion placed caution tape around play equipment until able to prevent children from playing on equipment.

746.1305(b) - Pre-service Training for Caregivers of Children Under 24 Months

Two staff were lacking their pre-service training of SIDS, Shaken Baby Syndrome, abusive head trauma and safe sleep practices.

746.3819 - Food Allergy Emergency Plan Signed by Parents and Health Care Professional

A review of children's files one child with a food allergy was lacking a parent signature along with symptoms to observed to administer medication and another child with a food allergy was lacking a food allergy plan.

746.603(a)(4) - Children's Records - Immunizations

Three of ten children's immunizations records reviewed were missing their all their required immunizations based on their age. A list was left with the director of children and immunizations the child was lacking.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

This standard was reviewed as part of an investigation and found non-compliant. It was discovered that a three year old child left the playground unsupervised and was located in front of operation by a parent.

746.701(1) - Licensing Incident/Illness Report Form - Injuries Requiring Medical Treatment or Hospitalization

This standard was reviewed as part of an investigation and found non-compliant. The incident report provided was lacking information required by licensing. The following was lacking on incident report: Director's name, operation number, if parent and licensing were notified (date, time, and by who), signature of director verifying they reviewed incident report and parent/guardian acknowledgement.

746.4907(d) - Installation and Maintenance of Loose-fill Surfacing - Proper Depth Maintained

It was observed that the operation was lacking proper depth of loose fill on the playground.

746.3819 - Food Allergy Emergency Plan Signed by Parents and Health Care Professional

It was observed that a child with a food allergy was lacking an emeregency plan on file.

746.2407 - Infant Care Area Furnishings and Equipment -Use of Safety Straps

It was observed that four of the six bucket highchairs were lacking safety straps.

746.3505(f) - Diapering Procedures-Soiled Diapers Kept in a Sanitary Manner

A soiled pull-up was observed in a trash can without a lid. The soiled pull-up was also not in a sealed plastic bag.

746.2415(b) - AP Cribs - Only Snug Fitting Sheet and Crib Mattress Cover Allowed if Infant Younger than 12 Months of Age

An infant under 12 months of age was observed in a bouncer that was placed inside a crib sleeping. The child also had a pacifier with a pacifier attachment and a bib on while sleeping in the bouncer that was placed inside the crib. A second infant under 12 months of age was observed in a crib sleeping with a boppy pillow. This was corrected at inspection as the bouncer was removed from the crib and the infant was removed from the bouncer and placed in the crib. The bib and pacifier were removed. The boppy pillow was also removed from the crib during inspection.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

There was no staff member or caregiver in a classroom supervising infants and toddlers who were napping when I walked into the classroom at inspection. This was corrected at inspection as the caregiver returned to the classroom.

746.2426(a)(1) - Infant May Not Sleep in a Restrictive Device

An infant was observed sleeping in a bouncer that was placed inside a crib. This was corrected at inspection as the child was removed from the bouncer.

746.901(4) - Required Personnel Records - Required Affidavit (Form 2985)

Two caregivers records lacked a notarized Affidavit for Applicants for Employment with a Licensed Operation or Registered Child-Care Home (Form 2985).
