745.8311(b) - Waiver/Variance - Comply w/ each condition while in effect

The operation was not following conditions on an approved variance. Per varaince condition an individual is to work in a classroom with a qualified caregiver. At inspection, observed the caregiver supervising three children 6 months to 11 months alone. This was corrected at inspection as a second caregiver was sent into the classroom.

746.501(a)(18)(C) - Written Operational Policies - Type of Physical Activity

The operational policies were evaluated and lacked written procedures for the following: A) The benefits of physical activity and outdoor play; B) The duration of physical activity at your operation, both indoor and outdoor; C) Each setting in which your physical activity program will take place; D) The criteria you will use to determine when extreme weather conditions pose a significant health risk that prohibits or limits outdoor play; and E) A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor play.

746.3701 - Safety - Areas Free From Hazards

Observed two children were each napping on a torn nap mat that had exposed foam in one classroom. Nap mats cannot be properly sanitized if torn and have exposed foam.

746.603(a)(3) - Children's Records - Health Statement

A sampling of children records were evaluated. One child's record lacked a health statement.

746.1613(a) - Reduction of child/caregiver naptime ratio by 50% allowed, If 13 or more children in care, children are 18 mo. or older and other standards met

In one classroom, during nap time, one caregiver was supervising 7 children, ages 15 months to 17 months. These numbers exceed the ratios allowed in a Licensed Child Care Center. This was discussed with the director. This was corrected at inspection as a second caregiver went into the classroom to assist supervise the children.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

One caregiver was caring for 5 infants birth through 11 months. This was corrected at inspection as a second entered the room. In another classroom one caregiver was supervising 6 children ages 14 months to 17 months. This was corrected at inspection as the second caregiver returned to the classroom.

746.1107(2) - Additional Minimum Qualifications for Caregivers - HS Diploma, Equivalent, Certification of Completion

This standard was reviewed as part of an investigation and found deficient. It was determined an individual has been working in a classroom with children as a caregiver who does not have a high school diploma or equivalent.
