746.3407 - Maintenance of Building, Grounds and Equipment

The following was observed: *The fabric on one of the play couch is showing signs of deterioration. The couch was worn. (Note: The director discarded the couch during the inspection). *Several classrooms require maintenance to refresh the wall paint on certain areas of the wall. *The carpeting in one of the classrooms is stained and needs to be cleaned. *A non-functional toilet was observed with visible bodily waste and tissue. The toilet requires maintenance and cleaning. *Three tricycles have worn-out handle grips that need replacing. The handle grips were torn. (Note: The tricycles were moved and made inaccessible to the children).

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

The operation did not have a current fire inspection report on file at the time of inspection.

746.3415(4) - Employee Handwashing After Diapering a Child

A caregiver did not wash their hands after diapering a child.

746.2428 - No Swaddling of Infant Unless Completed Sleep Exception Form on File

An infant was observed to be swaddled and asleep in their crib. There was no sleep exception form on file. (Note: This was corrected during the inspection when the caregiver removed the swaddle.

746.3417(3) - Children Handwashing After Toileting or Diapering

A caregiver failed to wash a child's hands immediately following a diaper change.

746.901(4) - Required Personnel Records - Required Affidavit (Form 2985)

One caregiver did not have a notarized affidavit for employmet on file at the time of inspection.

746.3505(f) - Diapering Procedures-Soiled Diapers Kept in a Sanitary Manner

The trash can being used in one of the classrooms had used diapers that were accessible to children. The trash can did not have a lid.

746.3701 - Safety - Areas Free From Hazards

The following hazards were observed during the inspection: *A bottle of Lysol Gel was observed on the back of a toilet in one of the children s restrooms, posing a potential safety hazard as it was within their reach. (Note: The caregiver removed the chemical during the inspection). *There was a bag of potting soil observed on the playground accessible to children. (Note: The bag of potting soil was removed during the inspections). . *There were several broken toys observed on the playground accessible to children. (Note: The broken toys were discarded during the inspection). *The padding around the splash pad is damaged leaving the concrete foundation exposed and creating a tripping hazard for children.

746.603(a)(4) - Children's Records - Immunizations

Two of the ten children's records evaluated did not have an updated immunization record on file.

746.3701 - Safety - Areas Free From Hazards

The dinosaur climber on the pre-k playground has been properly anchored. (Note: The operation will be removing the dinosaur climber). The concrete around the poles on one of the toddler playgrounds was exposed posing a hazard to children.

The play strcuture on the pre-k playground was removed. The loose fill surfacing around the dinosaur climber on the pre-k playground did not meet the 6 inch depth requirement. (Note: The operation will be removing the dinosaur climber).
