746.1309(e) - Documented Annual Training- Required Content

One out of the four employee records reviewed was missing the following required trainings for the annual year 10/7/2024-10/7/2025: building safety, emergency preparedness, communicable diseases, medication administration, and allergic reactions.

746.1315(a) - Each Caregiver and the Director Must Have Current Training in pediatric first aid with rescue breathing and choking.

Director is missing the current CPR/First Aid training which expired 6/1/2026.

746.303(b) - Report Number of Employees

A review of the Employees Who Left Employment in 2025 data from January 20, 2026, determined the center had not reported the number of employees who left employment in 2025 by January 15, 2026, as required.

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

The last fire inspection done at the center was on 8/11/2023 which expired on 8/11/2024.

One employee that was hired four months ago is missing the First Aid/CPR Training and had unsupervised access to children. Note: This was corrected during inspection by placing a caregiver with CPR/First aid Training in the room.
