746.3417(4) - Children Handwashing After Outdoor Activities

It was found that a caregiver did not take children to wash their hands after coming inside from outdoor activities.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

It was found that when transitioning into the classroom from being outside, a caregiver did not notice that a child remained in the outdoor play area, and the child was left unsupervised for approximately 8-10 minutes.

746.3407 - Maintenance of Building, Grounds and Equipment

During this investigation inspection there was a container catching a water leak under a sink in the kitchen that was found to be kept in an unsanitary manner. There were also oriental cockroaches found throughout the area. In addition, a container for refuse water under a portable sink was found to have mold inside of it.

746.3505(e) - Diapering Procedures - Sanitizing Diaper Changing Area

It was determined throughout the course of the investigation that a caregiver failed to sanitize a diaper changing area after diapering a child.

746.3415(4) - Employee Handwashing After Diapering a Child

It was determined throughout the course of the investigation that a caregiver failed to wash their hands after diapering a child.

746.3417(3) - Children Handwashing After Toileting or Diapering

It was determined throughout the course of the investigation that a caregiver failed to wash a child's hands after diapering.

746.2207(a) - Screen Time Activities - Not Used for Under Two

During an unannounced inspection, it was found that a child younger than 2yo was allowed to have screen time activities.

746.3701 - Safety - Areas Free From Hazards

During this investigation, I found a playground area to be noncompliant with safety hazards present.

746.1301(a)(2)(A) - Training Requirements for Employees, Caregivers, and Directors - Pre-Service Training

During a record audit, it was discovered that a new hire did not have all 24 hours of pre-service training completed within the first 90 days of employment.
