746.3417(3) - Children Handwashing After Toileting or Diapering

At the time of the inspection in Classroom 1 during diaper change, hand washing was not observed. NOTE: This was correct when the teacher assisted the child with hand washing.

746.3407 - Maintenance of Building, Grounds and Equipment

A board on the 3 year and up playground was observed to be loose. This was CORRECTED AT INSPECTION when the maintenance person resecured the board.

746.2419(6) - Requirements for Feeding Infants - Label Bottles/Cups

Bottles in the infant room were labeled with the initials only. This was CORRECTED AT INSPECTION when the bottles were appropriately labeled.

746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

A caregiver in the 4 year old room did not know how many children she had. The caregiver stated she had 9 children when there were 11 in care.

746.613(b) - Immunization Records, Exceptions, and Exemptions Current

Two of ten children's records reviewed did not have current immunization records in their file.

The side passenger door of a van used to transport children would not open from inside of the van and the back door of the same van would not open from the outside of the van.

A medication for a child in care stored at the center is expired.

A plastic climbing wall on the play scape is cracked in multiple areas.
