A child was observed sleeping with a blanket in the crib.

The classroom was observed with a blocked exit while a child napped on thier cot.

A staff member and the director did not have the requiredl transportation trainiing available for review during the inspection.

Upon arrival to the operation, there were 93 children present at the operation. The operation's capacity is 85.

Three children's records were lacking a preschool health statement

746.1311(d) - Director Annual Training - Caring for children under 24 months

The Director was lacking current training for children under 24 months of age. The operation cares for this age.

746.603(a)(4) - Children's Records - Immunizations

Two chidlren's records were lacking a current immunization record.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

One caregiver present during an inspection did not have a current background check. Caregiver was working in a room with children. The operation has not received complete eligibility before allowing the caregiver to have access to the children in care. Caregiver works on Fridays and Summers only. A background check has been submitted and completed.

746.303(b) - Report Number of Employees

A Review of the Employees Who Left Employment in 2024 data from January 22, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

The 18-23 month old room had 10 children with one caregiver. One caregiver may only have 9 children this age. This was corrected when one child was moved to another classroom.
