A notarized Affidavit for Employment - Form 2985 is needed for one caregiver.

It was observed that children's blankets were pilling on top of each other on a table.

It was observed the carpets in the two year old need vacuuming. There were dry leaves and dirt on the carpets where children play and take nap. It was observed in the after school room that there were sand and dirt on the floor by one of the entrance. This was corrected at inspection It was observed cracker crumbles and dirt on the floor by the trash can in the school age room. This was corrected at inspection.

There was an uncovered electrical outlet in the two year old room. This was corrected at inspection.

It was observed that there were sand and dirt on the changing table where children's diaper was change daily.

There was an uncovered electrical outlet in the two year old room.

It was observed that the air conditioning unit by in the back, where school age children use to enter the build, was not cover with a safeguarded.

It was observed that the floor in the schooler and the two year old class are dirty with grease, dry leaves, water spilled on the floor from lunch. It was also observed that broken and dirty bucket that contain toys in the two year old room.

It was observed that all of the children's coat and backpack were pilling on top of each other in the two year old class.

The center has not paid the December 2024 invoice for background check.

A child was placed in a restroom as form of discipline.

It was observed that trash bin in the Pre K - Pre school room and school age room did not have a lid and there are trash inside the bin.

It was observed that there four uncovered electrical outlets in the pre k - pre school room.

It was observed that the glass windown in the Two's year old class cracked and the operation used cardboard to cover it. This was cited two weeks ago.

It was observed that there was not enough loose-fill materials on the school age playground. The mulch was almost gone at the bottom of the climbing equipment. This was cited two weeks ago.

10/17/24 - Only one diploma was available to review at inspection. Still missing three. Proof of education is needed for four caregivers.

It was observed that children were outside playing with dirty cups and bows that flew out of an uncovered trash bin. This was observed when the Two's and Three's were outside on the playground. It was also observed an uncovered trash bin full of trash outside near the school age playground. This trash bin was accessible to children.

Four pre school children were missing copy of most current immunization record.

One pre school child was missing the health statement.

It was observed that there was not enough loose-fill materials on the school age playground. The mulch was almost gone at the bottom of the climbing equipment.

746.1315(a) - Each Caregiver and the Director Must Have Current Training in pediatric first aid with rescue breathing and choking.

The director and one caregiver have expired CPR and First Aid. Two other caregivers did not have a copy of current CPR and First Aid on file during inspection.

Proof of the current fire inspection was not available to review during inspection.
