Based on information gathered, a child was able to exit their classroom to the playground unsupervised on two occasions. Also, video evidence provided shows a caregiver videoing children and failing to intervene while children are seen climbing furniture.

Management failed to deliver form 2866 child interview letter to the parents of two children interviewed by Child Care Regulation.

Based on information gathered, a child was able to exit their classroom to the playground unsupervised on two occasions. These incidents were not reported to Child Care Regulation.

Based on information gathered through interviews and video evidence, it was determined that staff members have grabbed and pulled children.

Based on video evidence provided, a caregiver did not demonstrate good judgement when she recorded children when she was supposed to be supervising them and recorded them without the consent of the parents or guardians.

At time of inspection operation was observed with debrins in the classroom cabinets and diaper changing tables.

At time of inspection a toddler caregiver did not conduct handwashing after diaper change of a child. This was corrected at inspection.

At time of inspection, a toddler caregiver stated class had eleven children when they had twelve. This was corrected at time of inspection.

During inspection, the trashcan in the school-aged classroom did not have a lid on it and was used to dispose of food. Corrected at inspection.

At time of inspection, two out of ten staff files did not have pre-service certificate.

746.1315(a) - Each Caregiver and the Director Must Have Current Training in pediatric first aid with rescue breathing and choking.

At time of inspection, one out of ten staff files did not have a current CPR/First Aid certification.

At time of inspection, five out of ten children files did not have admission date.

At time of inspection, a diaper rash spray was observed in drawer at childs reach in childrens restroom.

During the inspection, the diaper changing station was noted to have debris present. During the inspection, it was noted that the classroom floors contained debris.

At time of inspection, two infants, seven and eight months were observed laying flat on the rug while drinking formula.

At time of inspection, two staff files did not have verification of completing operation orientation.
