746.1003(5) - Director Responsibilities - Direct Care for No More Than 10 Hours

During the DFPS investigation, it was found that one caregiver is working over 10 hours of direct care of children.

746.3505(g) - Diapering Procedures-Soiled clothes sent home

During a DFPS investigation, it was found that dirty clothes were not sent home immediately which resulted in the clothes being moldy once sent home.

746.2431(2) - Infant Daily Report - Times and Amount of Food

During a DFPS investigation it was found that the daily sheets did not accurately reflect the feedings of the infants in care.

746.1613(a)(5) - Naptime Child/Caregiver Ratio - Additional Employees Required

During an inspection during naptime, there was not an additional caregiver who was not counted in the child/caregiver ratio. This was corrected once naptime was over.

746.2426(a)(1) - Infant May Not Sleep in a Restrictive Device

Observed 2 infants sleeping in rocking chairs. This was corrected when the teachers were asked to put the sleeping infants in their beds.

746.2417(2)(B) - Activities for Infants - Not in Restrictive Device for more than 15 Minutes

Abserved multiple infants sitting in rocker chairs from 1;27 pm until 3:53. Licensing had told the caregivers at the start of the inspection that the infants can only be in the restrictive devices for 15 minutes and they failed to remove the infants throughout the entire inspection. This was corrected when the infant were removed from the devices.

746.2419(8) - Requirements for Feeding Infants - Other Use of Handwashing Sink

It was found that infant bottles are washed/rinsed in the sink in the infant room that is also used for handwashing. There was a bottle brush and dish liquid on the sink. I spoke with the caregivers about not washing bottles in their sink in the room.

746.1613(a)(6) - Naptime Child/Caregiver Ratio - Caregivers Physically Present

During an investigation it was found that a caregiver went on break. Another caregiver supervied two different classrooms during this time, walking back and forth to each classroom

746.1601 - Child/ Caregiver Ratio - 13 or More Children

Observed one teacher in the room with 9 infants. The ratio for that age group is 1 teacher per 4 infants. The other teacher was in the office inside the infant room on their lunch break with the door shut. This was corrected when the teacher came back into the room.

746.2419(3) - Requirements for Feeding Infants - Never Prop Bottles

Observed an infant (5 months) with a bottle propped with a stuffed animal. This was corrected when the caregiver saw licensing and removed the stuffed animal and picked the infant up.

746.2419(1) - Requirements for Feeding Infants-Hold through 6-months

Observed a 3 month old infant being fed in a rocking seat while the teacher held the bottle to the infants mouth. This was corrected when Licensing told the caregiver that infants 6 months and younger and any infant who cannot hold their own bottle must be held to be fed. The infant was taken out of the rocking chair to be held for the rest of the feeding.

746.3407 - Maintenance of Building, Grounds and Equipment

Observed nails and screws sticking out of the floor of the infant room. A screw was removed from the floor that was sticking up. This was corrected when duct tape was placed over the nails and screws. Observed an outside door that was rotted at the bottom and splintered. Observed brown water stains in the kitchen, classroom and bathroom areas. Observed a baseboard coming off the wall in a classroom.

746.2419(7) - Requirements for Feeding Infants - No Walking or Sleeping with Bottle or Cup

Observed an infant sleeping in a crib with a bottle. This was corrected when licensing asked the caregiver to remove the bottle.

746.2207(a) - Screen Time Activities - Not Used for Under Two

Observed children ages 19 months watching Ms. Rachel on a tablet. I spoke with the caregiver about children under the age of two not having screen time. The tablet was turned off during the inspection.

745.621(a)(2) - AP Initial background checks submitted - At the time you hire someone

During this follow up inspection it was observed that there was a person in a classroom with children without a background check on file. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078.

746.2419(7) - Requirements for Feeding Infants - No Walking or Sleeping with Bottle or Cup

Observed an infant in a crib with a bottle. The bottle was removed from the crib.

746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

One caregiver did not know how many children that they had. The caregiver stated that they should have 11 children, but there was only 10 children in the room. The caregiver counted and stated that one was moved.

746.1203(3)(D) - Personal use of electronic devices, such as cell phones, MP3 players, tablets, and video games

Observed a caregiver using thier phone for Tik Tok. This was corrected when the caregiver put the phone away.

746.3407(12) - Mark Cleaning Supplies, Toxic Materials; Keep Separate from Food and Inaccessible to Children

Observed Lysol and disinfectant spray under an unlocked cabinet in a bathroom that is used by the children in care. This was corrected when the cleaning supplies were removed and placed out of the reach of the children.

745.621(a)(2) - AP Initial background checks submitted - At the time you hire someone

There was two people at the operation without a cleared background check. One person was at the operation for a few hours to answer phones and the other person has been at the operation a couple of days to wash dishes. An administrative penalty will be imposed in the amount of 100% of the daily maximum per statute, HRC 42.078(a-2).
