746.2421(b) - Written Feeding Instructions - Review and Update Every 30 Days

Three infants were found to not have a valid written feeding instructions.

746.1311(e) - Director Annual Training - Additional required topics

The director did not have a current adminstering medication training in November of 2025 when a staff member administered medication to a child in care.

746.3805(c)(4) - Medication Records - Name of Employee Who Administered

A medication log was found to not be complete as the employee's name was missing.

746.2424 - Infants sleep in Designated Crib, Cot, Bed, or Mat

During inspection, an 6 month old infant who is not yet walking was observed sleeping in a blanket and not in designated crib. Note: This was corrected during inspection when director moved the infant into the crib.

746.2421(a) - Written Feeding Instructions - Signed and Dated by Parent or Health-Care Professional

Three out of the four infants did not have updated feeding sheets.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

During inspection, a individual who did not have an active background check was present alone in a classroom. This was corrected at inspection when the caregiver left the operation.

746.2405(2) - Infant Care Area Furnishings and Equipment - Crib for Each Non-walking Infant Younger Than 12 Months of Age

A non-walking 11 month old was sleeping on a mat.

746.2428 - No Swaddling of Infant Unless Completed Sleep Exception Form on File

A caregiver put down an infant that was swaddled to rest on the on boppy. Note: This was corrected when the blanket was removed.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

Two individuasl, who were being counted in ratio, did not have a background check submitted. Noted: The two individuals left the operation while I was present.

746.2207(c)(1) - Screen Time Activities - Related to Planned Activities

A child was using their personal phone for social media purposes and not educational purposes. Note: This was corrected when the child was asked to put their phone away.

746.1203(3)(D) - Personal use of electronic devices, such as cell phones, MP3 players, tablets, and video games

One caregiver caring for children was on a personal call during my walk through. Note: This was corrected when caregiver ended the call and put their phone away.

746.2415(b) - AP Cribs - Only Snug Fitting Sheet and Crib Mattress Cover Allowed if Infant Younger than 12 Months of Age

An infant was laying on a pillow inside the crib. This was corrected when the director removed the infant from the crib and was placed on the carpet.
