A caregiver did not have information readily available that showed the ages of the children present in her care. The caregiver did not know the last name of a child in her care and did not have this information readiy available in the classroom. This was corrected when the permit holder was notified and sent the information to the caregiver by phone.

746.1205(a)(3) - Responsibilities of Caregivers - Know children's names and ages

The caregivers in the infant room did not know the age of the oldest child and did not have information on hand showing the child's age.

746.1605(1) - Child/Caregiver Ratio - 18 Months Difference in Age

Three infants between 12-17 months old were observed combined with children who were more than 18 months older than them. This was corrected when it was addressed with director and the three infants were moved to the infant room.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

The director was observed alone with 15 children in a classroom. No other caregivers were present in the building. The specified age of the group was 2 years old meaning the director was out of ratio by 4 children. This was corrected when a second caregiver arrived about 30 minutes later and they moved the infants to their assigned classroom.

746.1605(1) - Child/Caregiver Ratio - 18 Months Difference in Age

Three year old children were observed in a group of children with four infants (2, 5, 12, & 16 months). The three year olds were more than 18 months older than the youngest children in the group. This was corrected when a second caregiver arrived about 30 minutes later and the infants were moved to their assigned classroom.

Two children did not wash their hands before eating the breakfast they were served.

A 2 month old was observed strapped in a carseat in a classroom. The infant was in the carseat for over 15 minutes after being dropped off.
