It was determined in an investigation that the chairs utilized by toddler-aged children were not age-appropriate, which resulted in a child falling and sustaining a bump to their head.

It was observed that a portion of the fence was not secure between the big playground and the basketball playground, allowing children to put their bodies through it. Additionally, on the basketball playground, a section of the fence leading to the other side, where the apartment complex is, was also not secure, permitting children to climb through to the other side.
