The operation was not maintaining records of the emergency drills. This was corrected at inspection when form 7263 Emergency Practices was provided to the operation and the drills were entered.

747.1309(d) - Primary Caregiver Annual Training - Required Special Topics without specific hours

The primary caregiver did not have proof of training in administering medication and preventing and responding to emergencies due to food or allergic reactions.

747.2329 - Covering Infant's Head or Crib

An infant was observed sitting in a car seat carrier with thie carrier covered by a blanket. The blanket was propped up by the handle. This was corrected at inspection when the blanket was removed.

747.4303(b) - Cots, Beds, Mats Labeled With Child's Name

Mats were not labeled.

745.621(a)(7) - AP Initial background checks submitted - At the time you become aware of anyone requiring a background check under 745.605

Five children were seen supervised by a person without a background check initiated with the operation. Note:This was corrected when the Primary Care provider returned to the operation.

747.203 - Primary Caregiver's Absence - Qualified Caregiver in Charge

Five children were found to be supervised by a non-qualified caregiver. Note: This was corrected when the caregiver returned to the operation.

747.3501 - Safety - Areas Free From Hazards

The playground was seen during an inspection with hazards present such as a post digger, household paint, and packaged hygiene items stacked against a wall of the house that could topple over. The yard also has many large branches present and the grass requires yard maintenance due to the height. The fence gate is damaged, one of the pickets is broken creating a hole in the gate.

747.2105(a) - Screen Time Activities - Not Used for Under Two

A 4 month old infant was present during screen time use. Note: This was corrected when the TV was turned off.

747.3501 - Safety - Areas Free From Hazards

The playground was seen during an inspection with hazards present such as a weed eater, post digger, dirty cat litter box, and household paint all in areas accessible to children in care.

A trampoline more than 4 feet in diameter is present on the playground during an inspection.

747.2105(c)(1) - Screen Time Activities - Related to Planned Activities

A 7 year old child is seen during an inspection watching cartoons that is not related to planned activities.

747.2405(1) - Furnishings and Equipment for Toddlers - Age-appropriate

The provider does not have an age appropriate nap mat for the 2 year old child in care available for viewing during an inspection.

747.4603 - Measurement of Use Zone for Stationary Equipment

The playground has 3 pieces of equipment that require use zones that have inadequate space around the equipment for the use zone. 2 slides and one climbing dome.

747.2315(b) - AP Cribs - Only Snug Fitting Sheet and Crib Mattress Cover Allowed if Infant Younger than 12 Months of Age

A 3 month old infant was seen sleeping in a crib with a blanket wrapped around them, clean diapers present, and a napkins present.

747.1309(c) - Primary Caregiver Annual Training - Special topics for children under 24 months

The provider does not have current training in Infant safe sleep, early childhood brain development, or preventing shaken baby syndrome available for review during an inspection.
