Observed a 7 month old infant with a blanket in the pack and play. This was corrected whent he blanket was removed. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078.

Observed a 7 month old infant sleeping with a bottle. This was corrected when the bottle was taken out of the crib.

An infant was found asleep in a car seat while in care. This was corrected when the operation staff was asked to remove the infant from the car seat.

Observed the television on and all children were watching paw patrol. I explained that children under 2 cannot have any screen time and the television was turned off.

The operation had not completed a Sheltering drill or documented that one was completed

The operation did not have the children product safety report for review. A form was emailed to the operation.

The operation could not provide proof of any training for the caregiver for year 2024-2025.

Observed 4 children records out of 11 children and none of the children had health statements

The operation did not have proof that a fire drill had been completed. There was no form to review. A fire drill was completed during the inspection.

Observed 3 adults in the home that do not have cleared background checks to be around the children. One caregiver was asked to leave the facility during the inspection. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078.

There was one infant that did not have feeding instructions for review.

The primary caregiver could not show proof of 30 hours of annual training. There were two training found but both of the trainings had been tampered with and could not be used as training.

747.2317(2)(B) - Activities for Infants - Not in Restrictive Device for more than 15 Minutes

Observed an infant that had been in the car seat for 2 1/2 hours. This was corrected when the infant was removed upon request from Licensing.

747.4403(4) - Prohibited Equipment - Rings,Trapeze Bars, Free-Swinging Ropes

Observed swinging bars on the playground equipment. I spoke with the primary caregiver about it being a prohibited item for the children.

747.5005(1) - Fire Drill Monthly - Exit in 3 Minutes

Operation was unable to find fire drill, shelter in place drill form.

CPSC document was not viewed at the operation. Document was sent to operation, but was unable to print it out.
