Three children did not have a complete FARE form for a diagnosed allergy as they were missing the required medication as prescribed by the physician.

One child's admission paperwork did not have the name, address, and telephone number of the child's physician or an emergency-care facility listed. This was corrected at inspection when the parent completed the admission paperwork with this information.

One classroom had a blocked exit with a nap mat for a child to nap on. This was corrected at inspection when the caregiver relocated the nap mat and the child to a different area in the classroom.
