Operation did not have a current consumer product services form 2885 available for review. Posted form 2885 expired in 2023. This was corrected at inspection when the director checked the CPSC web site, completed a new form 2885 and posted it.

Operation has eight employees listed in the person's list in CLASS that need to be invalidated.

746.5207(a)(1) - Emergency Evacuation and Relocation Diagram - Floor Plan

Two classrooms did not have an emergency evacuation plan posted. This was corrected at inspection when the caregivers posted the plans in the classrooms.

746.801(8) - Required Records Maintained and Made Available - Playground Maintenance Records

Operation did not have a playground maintenance checklist available for review.

746.1309(e) - Documented Annual Training- Required Content

In review of a sample of records, three employees do not have proof of annual training in food allergy and hazardous material available for review. Two employees do not have proof of training in emergency preparedness available for review.

746.4505 - Storage for Individual Belongings

During walk through of the two year old classroom, the storage/cubbies for children's belongings were not labeled.

746.4003(a)(8) - First Aid Kit Incomplete - Tweezers

The first aid kits in the two year old and Pre-K classrooms were missing tweezers. This was corrected at inspection when the director placed a set of tweezers in each first aid kit.

746.1309(e) - Documented Annual Training- Required Content

In reviews of records, the director is missing 4 of the required 30 hours of training in emergency preparedness, administering medication, how to address an allergic reaction and how to dispose of hazardous materials.
