746.5307(a) - Inspect Fire Extinguishers Monthly

The operation did not inspect the fire extinguishers for the month of March. Noted: This was corrected during the inspection when all the fire extinguishers were inspected and was documented on form 7263.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

A 14 year old who is regularly present at the operation does not have an active background check. This individual is transported to the operation with the enrolled school age children.

746.5607(e) - Safety restraint devices for child used when vehicle is on and during all time vehicle in motion

In an investigation, it was determined that four children,to children four years of age, and two children 10 years of age, between 50-54 inches, were not utilizing a safety restraint device while being transported from school to the LCCC by operation staff.

746.301(7) - Notification of Additional Services Offered

The operation offered a field trip to the school age children on February 9th. Noted: this was corrected during inspection when the service was added and requirements were discussed with staff.

746.5619 - Plan to Handle Transportation Emergencies

It was determined in an investgiation that the operation's plan for transportation emergencies did not include necessary information, such as how to handle emergency breakdowns and accidents, including vehicle evacuation procedures, supervision of the children, and contacting emergency help.

746.1203(3)(D) - Personal use of electronic devices, such as cell phones, MP3 players, tablets, and video games

It was determined in an investigation that the operation staff uses personal devices to make phone calls when transporting children.

746.3401(a) - Annual Sanitation Inspection

The operation did not have a current sanitation (health) inspection.

746.2405(4) - Infant Care Area Furnishings and Equipment - Hand-washing Sink

The infant space did not have a handwashing sink.

746.501(a)(24) - Written Operational Policies - Emergency Preparedness Plans

The emergency preparedness plan that was provided was missing the following items; How children will be evacuated or relocated to the designated safe area or alternate shelter, the staff responsibility in a sheltering/lock-down emergency for the orderly movement of children to a designated location within the center where children should gatherinformation regarding how children in attendance at the time of the emergency will be accounted for at the designated safe area or alternate shelter, information regarding how the operation staff will communicate with local authorities (such as fire, law enforcement, emergency medical services, health department), parents, and CCR staff. It also did not include how your staff will evacuate and relocate with the essential documentation, including: Parent and emergency contact telephone numbers for each child in care, Authorization for emergency care for each child in care, and the child tracking system information for children in care and did not include information regarding how staff will continue to care for thechildren until each child has been released and how operation staff will reunify the children with their parents as the evacuation, relocation, or sheltering/lock-down is lifted and informaton regarding how staff will contunie to care for children until each child has been released.
