This standard was tasked as part of an investigation and found deficient. Children younger than 12 have sat in the front passenger's seat when being transported by the operation.

746.1316(a) - Transportation Safety Training -Director and Employee Complete 2 Hours Annually

This standard was tasked as part of an investigation and found deficient. Two bus drivers do not have current transportation training.

746.5613 - One Seat Belt Per Person

This standard was tasked as part of an investigation and found deficient. On at least one occasion, children have shared safety belts due to the exceeded capacity of the center's van.

746.2207(b) - Screen Time Activities - May Supplement Activities Not Replace

This standard was tasked as part of an investigation and found deficient. Cartoons and Youtube videos are routinely shown in the afternoons and are not related to planned activities.

746.5617(a)(5) - Required Equipment in Vehicles - Fire Extinguisher

This standard was tasked as part of an investigation and found deficient. One vehicle used to transport children is not equipped with a fire extinguisher.

This standard was tasked as part of an investigation and found deficient. One vehicle used to transport children is not equipped with a first aid kit.

There was not a working carbon monoxide detector at the time of inspection.

The center director did not ensure an individual asked to assist in transportation had a background check ran.

Screen time was being used in an infant classroom with children under the age of two years old.

There was not a working carbon monoxide detector at the time of inspection.

While evaluating 10 children's files, I observed the following: One child did not have healthcare information or emergency care authorization. Two children did not have School Information. One Child idd not have a Special Care Statement, Health Statement or Immunization Information. One Child needed an updated immunization record.

746.3503(a) - Diaper Changing Equipment - Smooth, Non-absorbent, Easy to Clean Surface

During the inspection, I observed two diaper changing tables to have cloth covers. NOTE: During the inspection, the cloth covers were removed to leave only the plastic mat.

746.2417(2)(B) - Activities for Infants - Not in Restrictive Device for more than 15 Minutes

During the inspection, I observed three children to be present and strapped into a high chair table for over 20 minutes. NOTE: During the inspection, the children were removed from the chairs.

746.201(4) - Designating Director

During the application inspection, it was observed that there is not a qualified director in place.

During the inspection, I observed one infant teacher to be present with 6 children. The Specified Age Group of 12m allows for 5 children, and the classroom was over ratio by 1 child. NOTE: During the inspection, an additional caregiver arrived, bringing the class into ratio.
