746.801(13) - Required Records Maintained and Made Available - Gas Inspection Report

I did not observe the most recent gas pipe test available for review.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

Based on the information obtained during the DFPS investigation, it was determined caregiver did not provide adequate supervision when child in care walked out of the daycare unsupervised.

746.1205(a)(3) - Responsibilities of Caregivers - Know children's names and ages

During the inspection a caregiver did not provide the correct ages of the children twice when asked. This was corrected when the staff reviewed the role sheet to verify ages. Also the owner implemented staff writing the children's names and ages in a notebook daily.

746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

During the inspection a caregiver provided the wrong amount of children they were responsible for. This was corrected when the caregiver counted the children again and provided the correct amount.

746.603(b) - Children's Records - Storage

The operation could not provide admissions files for children who were enrolled within the last 3 months. (August 2025)

746.3407 - Maintenance of Building, Grounds and Equipment

There were three toilets that were not working properly during the inspection. This was corrected when the operations maintenance person fixed two toilets and provided the plan to replace the third one outside of business hours.
