745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

This standard was evaluated as a part of a DFPS abuse/neglect investigation and was found to be deficient. A person who was eligible to be present as a volunteer who may not be left alone with children was in charge of a group of children alone, during the time of the incident.

746.201(9) - Complying with Laws and Rules

This standard was evaluated as a part of a DFPS abuse/neglect investigation and was found to be deficient. The permit holder did not ensure that staff complied with laws and rules, which led to a child being left outside on the playground and then taken from the playground by an outside adult before being returned by law enforcement.

746.1003(2) - Director Responsibilities - Employees Comply with Standards

This standard was evaluated as a part of a DFPS abuse/neglect investigation and was found to be deficient. The director did not ensure that the employees complied with Minimum Standards as the director left the facility prior to ensuring that caregivers knew which children they were responsible for. The director also did not ensure that a volunteer was not left alone and in charge of a group of children.

This standard was evaluated as a part of a DFPS abuse/neglect investigation and was found to be deficient. A caregiver did not know which children they were responsible for.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

This standard was evaluated during a DFPS abuse/neglect investigation and was found to be deficient. A child was left outside and was unsupervised for 48 minutes without the caregiver's knowledge.

746.5205(1) - Fire Drill Monthly - Exit in 3 Minutes

The operation had no recorded fire drill that showed the operation was able to evacuate the children in under three minutes.

746.4907(a)(1) - Installation and Maintenance of Loose-fill Surfacing - Six Inches if Equipment 5 Ft or Less

During the inspection, the loose fill on the playground was either missing or present at no more than 2 inches deep in the use zones of both play structures.

746.2911 - Adequate Lighting During Naptime

During the inspection, the two and three year old room was not adequately lit for nap.
