It was determined that staff was not actively supervising children at the time of incident.

It was learned that a child sustained an injury while care which required medical treatment. The incident was not reported to Licensing as required.

HRC42.041(a) - Illegal Operation - Not Licensed

It was determined that the operation was providing care for children on 9/15/25. The initial permit was issued on 9/26/25.

744.2051(8) - Space and Equipment - System for Belongings

At the time of inspection there was no system available for storing children's personal belongings. Backppacks were on the floor along a wall in the cafeteria without tags identifying the program.
