A child was left unsupervised in the cafeteria for approximately 5 minutes.

744.1601 - Child/ Caregiver Ratio

Based on interview statements, it was determined that the operation has been out of ratio with one caregiver at times having up to 30 children, when the maximum to one caregiver is 26.

744.603(a)(2) - Children's Records - Admission Information

It was determined the operation did not have hard copy enrollment forms for children present. While enrollment information is available by tablet the reception at the location is not always accessible. The staff need to be able to access the student's enrollment information as needed for food allergy information, parent contact information, list of authorized release people, and more.

At the time of inspection the operation had not conducted any monthly Fire Drills.

There were children in care with a food emergency plan on file but the medication stated that was needed by health care provider in the event of an allergic reaction was not available at the operation.

During the inspection a child had their epi pen in their backpack which is accessible to other children.

744.2669 - Food Allergy Plan-Emergency plan for diagnosed child, signed by health care professional and parent

During the inspection one child with food allergies did not have a completed food emergency plan.
