The toddler and preschool playground was observed to have water on the fixtures causing a hazard for the children playing on them. This was corrected at inspection when the caregiver wiped the fixtures down with a towell.

1 of the10 caregivers files reviewed was missing a required training certificate for Preventing and controlling the spread of communicable diseases, including immunizations.

The operation's bus binder did not have emergency authorization forms available for review. This was corrected at inspection when the operation made copies of the children's emergency authorization forms and placed them in the binder.

This standard was tasked to be reviewed as part of an investigation and is found to be deficient.

This standard was investigated as part of an DFPS investigation and found to be in non-compliance. Based on evidence obtained during the investigation, it was determined that a caregiver did not use good judgement when laying children down by grabbing them by their arms and laying them down roughly posing the risk of a child being injured.

This standard was investigated as part of an DFPS investigation and found to be in non-compliance. Based on evidence obtained during the investigation, prohibited punishments was used when an employee was observed grabbing children by the arms to lay them down during naptime.
