During Inspection, it was discovered that the fire inspection had expired.

During the walkthrough of the garage, a hammer was observed to be accessible to the children in care. This was corrected by locking the hammer in the storage room located next to the employee bathroom.

The service plan reviewed was missing the signatures of the service planning team members that were involved in the development of the service plan, and it was also missing the effective date of the service plan.
