746.2415(b) - AP Cribs - Only Snug Fitting Sheet and Crib Mattress Cover Allowed if Infant Younger than 12 Months of Age

During the follow up inspection a 7 month infant was observed asleep with a blanket in the play pen. This was corrected at inspection when the blanket was removed from the play pen.

746.3407(9) - Floors, ceilings and Walls in Good Repair and Clean

Food and trash were observed on the dining room floor and kitchen floor. This was corrected at inspection when the director began cleaning the area.

746.3407 - Maintenance of Building, Grounds and Equipment

There was an highchair placed outside and forgotten about instead of discarded. There were also bags of dirty diapers left on the highchair. Furthermore the grass on the outdoor playground needs to be mowed. Inside the center, dead bugs in window seals and on the floor of the bathroom were observed.

746.5205(1) - Fire Drill Monthly - Exit in 3 Minutes

The operation has not conducted a fire drill since they opened on August 4, 2025, leaving the operation without a fire drill in the month of August. This was corrected at inspection when the operation conducted a fire drill.

746.2407 - Infant Care Area Furnishings and Equipment -Use of Safety Straps

The kidney bean table being used by children for lunch time was missing the required safety straps. This was corrected at inspection when the children were removed from the table and placed at a toddler sized table instead.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

A caregiver was present at the operation without an active background check. This was corrcted at inspection when the caregiver left the operation.
