A caregiver did not wash a infant's hands after diaper changing. The caregiver also did not wash her own hands.

746.4901(a) - Active Play Surfacing In Use Zones - Loose Fill or Unitary Surfacing

There was a slide on the playground that did not have the required six inches of surfacing underneath it. The slide was removed at the inspection.

The operation did not have documentation of conducting lock down, shelter, and weather drills.
