745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

A person was present and caring for children without receiving notification from the CBCU that the person is eligible to be present. This was corrected during the inspection when the person left the building.

746.305(a)(2) - AP Report Child Injury Requiring Medical Treatment by a Health-Care Professional or Hospitalization

This standard was evaluated during the investigation and was found to be deficient. The operation did not report the injury to CCR.

746.3301(b) - Nutrition and Food Service - Follow Meal Patterns

The operation's weekly menul was missing the following componets: Breakfast- Grain- two days, Lunch- Meat or meat alternative- two days, Lunch- Fruit- one day.

746.4913 - Required Documentation for Unitary Surfacing Materials

The operation does not have unitary surfacing documentation on file.

During the inspection two caregivers were present and caring for children without the operation receiving notification from the criminal background check unit that they were eligible to be present and caring for children. This was corrected when the caregivers left the building during the inspection.

745.635(3) - AP Background ck determination - If provisionally eligible w/conditions or eligible w/conditions you must restrict duties per conditions/restrictions

The background check status of a caregiver includes a condition which states that this person may only be supervised by a person with no conditions regarding his or her presence at the operation. During the inspection the caregiver was caring for children in a classroom along with another caregiver who did not have an eligible background check status. This was corrected during the inspection when a caregiver with an eligible background check status replaced the caregiver without an eligible background check status in the classroom.

746.5207(a)(2) - Emergency Evacuation and Relocation Diagram - Two Exits from Each Room

This standard was found to be defieicent after the operation did not have emgency evacuation diagrams with two exit paths from each room during the inspection.

746.4503(b) - Cots, Beds, Mats Labeled With Child's Name

This standard was found to be deficient after the operation's sleeping cots were not properly labeled or identified.

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

This standard was found to be deficient as the operation was not able to provide a current fire inspection during the application inspection.

746.3407 - Maintenance of Building, Grounds and Equipment

This standard was found to be deficient after it was observed that a toilet rail in a classroom needs repair. Additionally, the playground area has an outdoor faucet box missing a cover. Loos materials were stored near an entrance , creating a tripping hazard, and a ceiling tile was missing in a classroom , exposing pipes and wiring.

746.2403(2) - Infant Care Area - Safe, Open Floor Space

This standard was found to be deficient as both infant classrooms were not set up to include safe, open floor space for floor time play during the inspection.
