746.5617(a)(2) - Required Equipment in Vehicles - Emergency Medical Information

The transportation clipboard used to pick up school-age children was missing copies of each child's medical authorization information. NOTE: Copies of the medical authorization forms from each child's enrollment file was added to the clipboard.

746.5617(a)(5) - Required Equipment in Vehicles - Fire Extinguisher

Both vehicles had expired fire extinguishers from April 2026.

746.5603(c) - Types of Vehicles Used to Transport Children - Be Maintained Safely

A seatbelt in the vehicle did not fully retract. The second vehicle was not operable due to needing a battery.

746.3407(8) - Garbage Inaccessible to Children; Maintained in Sanitary Manner

A trashcan that contained soiled diapers did not have a lid in one restroom. NOTE: The trashcan was removed during the inspection.

746.701(5) - Licensing Incident/Illness Report Form - Situations Placing Children at Risk

The operation did not complete an Incident/Illness Report form, 7239, documenting the incident between a caregiver and a child.

746.305(a)(6) - Report Situation Placing Children at Risk

The operation did not self-report an incident involving a caregiver and child to Child Care Regulation within two days.

746.2805(3) - Prohibited Punishments - Grabbing and Pulling

Based on the investigation findings, a caregiver was seen separating two children who were wrestling by lifting one child by their forearm onto their feed and remained holding the child's forearm while walking them to a nearby wall, then pulling the child's arm downward to make the child sit against the wall.

746.603(a)(9) - Children's Records - Medication Records

During the investigation, it was determined that a child was administered Tylenol due to a fever per the parent's request but there was no documentation kept by the operation showing the amount of medication, date and time administered, and name of the person who administered the medication.

746.305(a)(4) - Report Outbreak of Communicable Illness

During the investigation it was determined that the operation failed to notify CCR and the Health Department of reported cases of Salmonella at the operation.

746.307(d) - Parental Notification of Communicable Disease

During the investigation it was determined that the operation failed to notify parents of Salmonella at the operation within 48 hours of being notified. Parent notification was made six days after the first reported case and only to one classroom.

746.3407 - Maintenance of Building, Grounds and Equipment

During the investigation, it was observed that two crock pots were used to warm infant bottles for feeding. Both crock pots had white film floating on top of the water.

746.1301(a)(2)(A) - Training Requirements for Employees, Caregivers, and Directors - Pre-Service Training

Two caregivers with start dates of 5/27/25 had not completed their remaining 16 hours of pre-service training within 90 days.

746.3805(b)(1) - Administering Medication - Original Container with Child's Name, Date

A child with medication kept on site was not in the original prescription box.

746.3701 - Safety - Areas Free From Hazards

The cords for an Alexa device and power cord were hanging from the windowsill in the younger infant room. In the Jack-and-Jill restrooms between rooms 3 and 4, a small spray bottle labeled Fabuloso was found in one restroom. In the second restroom there was an unused floor lamp resting in the corner of the restroom and a container of moisture eliminator accessible to children. NOTE: This is a re-cite regarding hanging cords. The caregiver unplugged the devices and moved the cords during the inspection.

746.3501(6) - Diaper Changing Steps-Supplies Kept Out of Child's Reach

There was a diaper rash ointment and spray found on the floor of the restroom between rooms 2 and 3. NOTE: The caregiver was asked to place the items in their closet during the inspection.

746.3701(10) - Safety - Televisions Anchored to Prevent Tip Over

The television in the school-age room was observed sitting on top of a cabinet and not secured or anchored.

746.3701 - Safety - Areas Free From Hazards

Three classrooms were observed with electrical cords hanging at the level of the children. One room had an air purifier plugged in and was sitting on the floor. NOTE: During the inspection one room placed a shelf in front of the cord and a second room tucked the cord behind a nearby shelf. The air purifier was moved out of the reach of the children.

746.1301(a)(7)(A) - Training Requirements for Employees, Caregivers, and Directors - Transportation training

A child was transported to school by a driver who did not have a transportation training certificate on file. The director who is also a designated driver did not have a transportation training certificate on file.

746.4501(b) - Chairs - Safety Straps Used if Required by Manufacturer

Two infants were observed sitting and eating in bucket seats without the use of safety straps. NOTE: The caregiver placed the safety straps around the infants during the inspection.

746.2207(a) - Screen Time Activities - Not Used for Under Two

A classroom that contained a child under 2 years of age was observed watching a phonics video on an Ipad. NOTE: The Ipad was turned off and put away during the inspection.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

A caregiver without an entered background check was observed in direct care of children. NOTE: The caregiver was sent home during the inspection.

746.2703(1) - Space Requirements for School-age - Interest Centers

The classroom being utilized for School-age children did not have toys availble for children during the inspection.
