It was observed that the floors in the operation had not been mopped recently. There was debris on the floor such as dust and food crumbs and there were drawings and stains on the walls. There is a floorboard that is broken, the doorbell is missing on the front door, and there is damage to the wall.

During an initial inspection it was observed that there was an outlet in reach of children that had no outlet cover or panel in the wall. Leaving it exposed to children during transition times.

During a walk through of the facility it was observed that there were coins on the floor where the children walk to get to classrooms. There were cars with detachable wheels in a classroom with 18 month olds.

It was observed that the operation had smaller garbage cans without a lid on and a child was reaching into the trash can.
