After reviewing 4 child files, one child was missing immunization records and doctor statement.

746.901(4) - Required Personnel Records - Required Affidavit (Form 2985)

After reviewing 3 staff files, one staff did have have the notarized form 2985,

746.207(a) - AP Parent Notification of Liability Insurance - If No Insurance Must Notify Parent in Writing at Enrollment

During an inspection, it was observed parents were not notified in writting of the operation not having liability insurance. This was corrected when the owner filled out form 2962 and posted it.

746.1301(a)(2)(A) - Training Requirements for Employees, Caregivers, and Directors - Pre-Service Training

The two personnel files reviewed did not have pre service training.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

During the inspection there was a person present with direct access to the children before their FBI fingerprint results were in. The person was being counted into ratio.

A caregiver with a conditional background check was observed to be left in charge of the operation.

746.3407 - Maintenance of Building, Grounds and Equipment

The fence on the playground area had broken wood and exposed nails. There was also trash and broken toys on the playground area.

746.5205(1) - Fire Drill Monthly - Exit in 3 Minutes

There has not been a fire drill conducted at the operation since 04/28/25. This was corrected at the inspection when a drill was conducted and met the required exit time.
