A Merry Go Cycle in the outdoor playground was missing a pedal. This posed as a safety hazard as the metal part of the pedal was exposed and accessible to children. Note: This was CORRECTED at inspection as the Merry Go Cycle was removed from the outdoor playground.

A caregiver's purse was observed on top of a children's shelf. This posed a safety hazard as the caregiver's purse was within children's reach. Note: This was CORRECTED during the inspection as the purse was stored in an area out of children's reach.

746.5607(b) - Child Passenger Safety Seat System - Not expired or damaged in accident

1 of 3 booster seats was expired in one of the transportation vehicles. Note: This was CORRECTED during the inspection as the expired booster seat was removed from the vehicle.

746.401(5) - Posting Requirements - Activity Plans for each group of children in the child care center

There were no activity plans posted in a toddler room and afterschool rooms. The activity plans posted in the infant room and pre-k room were not current.

746.1605(1) - Child/Caregiver Ratio - 18 Months Difference in Age

Two infants were combined with older children in a toddler room. There was more than an 18 month age difference between the youngest and oldest child in the group. The youngest child was 15 months old and the oldest child was 3 years old.

746.1613(a)(4) - Naptime Child/Caregiver Ratio - Additional Employee Required

There was no additional employee present in the building during the naptime.

746.2207(a) - Screen Time Activities - Not Used for Under Two

Upon arrival to the operation a group of children of varying ages including a 14 month old infant were observed watching a movie during the naptime.

746.2426(a)(2) - Infant May Not Sleep in a Restrictive Device - Remove the infant from the device and place the infant in a crib as soon as possible

A 4 month old infant was observed asleep in a restrictive d evice (swing) for an undetermined amount of time. Note: This was addressed with the caregivers during inspectoin and the infant was moved to their assigned crib.

There was more than an 18 month age difference between the youngest and oldest child in a group when an infant was combined with older children. The infant was 14 months of age and the oldest child in the group was 10 years old.

Three infants were left unsupervised for an undetermined amount of time when the caregiver in charge of the group of children was observed in the office area with two other employees upon CCR s arrival to the operation.

746.1301(a)(3)(A) - Training Requirements for Employees, Caregivers, and Directors - Pediatric First Aid with Rescue Breathing

A caregiver caring for a group of children did not have training in Pediatric first aid with rescue breathing.

A hole was observed on a wall leading to a toddler room. A door to a storage area was broken off in the outdoor playground.
